In order to download your PDF you must meet the following requirements:

1. Minimum Web Browser Requirement.

This site requires a minimum Web browser of Microsoft Internet Explorer 6.0 or higher, Mozilla Firefox 1.5 or higher, or any other Web browser compatible with these browsers.

If you are not sure of the version of your browser, select Help in the menu bar of your browser and choose About.

2. Adobe Acrobat Reader 6.0 or higher is required to view and print the PDF.

If you are not sure of your version of Acrobat Reader, select Help in the menu bar while inside Acrobat Reader and choose About.

3. Valid email address (to receive confirmation email).

4. Access to a printer  (that is loaded with paper and has ink or toner).
